To Schedule a NEW appointment
Please complete the Appointment Request Form. This can be completed on a laptop, desktop computer, and/or a mobile device.
All fields must be completed, including insurance information.
We are in-network providers for Blue Cross/Blue Shield, UPMC HealthPlan, and Medical Assistance (CCBH and CBH). We are NOT in network for UnitedHealthcare, Aetna, Cigna, WPEE, or Tricare.
We must be in-network with BOTH your primary insurance AND secondary insurance in order to bill Medical Assistance.
You will receive a call or email within one week of receiving your request form with your scheduled appointment time.
Prior to your scheduled appointment
Please review the Client Information for Assessments and our Fees for Services.
Three weeks prior to your scheduled appointment, you will receive a confirmation call or email. You MUST confirm your appointment or it may be cancelled.
Once your appointment is confirmed, you will receive consent, releases of information, and history forms VIA EMAIL that must be completed prior to your appointment.
