To Schedule a NEW appointment

  • Please complete the Appointment Request Form. This can be completed on a laptop, desktop computer, and/or a mobile device.

  • All fields must be completed, including insurance information.

    • We are in-network providers for Blue Cross/Blue Shield, UPMC HealthPlan, and Medical Assistance (CCBH and CBH). We are NOT in network for UnitedHealthcare, Aetna, Cigna, WPEE, or Tricare.

    • We must be in-network with BOTH your primary insurance AND secondary insurance in order to bill Medical Assistance.

  • You will receive a call or email within one week of receiving your request form with your scheduled appointment time. 

Prior to your scheduled appointment

  • Please review the Client Information for Assessments and our Fees for Services.

  • Three weeks prior to your scheduled appointment, you will receive a confirmation call or email. You MUST confirm your appointment or it may be cancelled.

  • Once your appointment is confirmed, you will receive consent, releases of information, and history forms VIA EMAIL that must be completed prior to your appointment.

  • Questions or concerns about EXISTING APPOINTMENTS can be sent to your scheduled clinician. Clinician email addresses are on our providers’ page.
